Support at Home replaced Home Care Packages in November 2025. Care management includes planning, coordination, reviews and support; provider billing and administration are separate functions. Read about care management on My Aged Care.

Support at Home and aged care case management

Seeking aged care services can bring up questions, changing needs and unexpected concerns. We help coordinate the next steps and work through issues as they arise, easing the load on the loved ones who support you.

We help you review services in light of your assessed needs and the relevant Support at Home program guidelines, and ask questions about provider qualifications and requirements before decisions are made.
